Who updates/provides all the public information on this website (blogs, etc.)?

All information provided and updated on this website is currently created, edited, and updated by Kathryn M Eddingsaas, RD, CD, CEO.

Who provides services such as the meal plan package, 1:1 medical nutrition therapy, outpatient nutrition therapy, and eating disorder therapy?

All services provided through Simple City Wellness are currently done by Kathryn M Eddingsaas, RD, CD, CEO.

Where can services be provided? Can they be provided virtually?

Currently, Kathryn Eddingsaas is working and certified and licensed as an RD in Wisconsin. All services are currently virtual. Kathryn Eddingsaas, RD, CD, is only practicing with patients in Wisconsin at this time. Kathryn plans on getting certified in more states to provide more access to virtual care. Proof of Wisconsin residency will be required before time of service.

Do the therapeutic/outpatient therapy services accept insurance?

At this time, Simply City Wellness does not accept insurance payments/plans. All services are paid out-of-pocket and prices on each service are listed as such. Payments must be made online ahead of service.

Simplecitywellness reserves the right, in its sole discretion, to change the terms under which its most recent version of the terms will supersede all previous versions. Simplecitywellness encourages you to stay informed of our updates. All current policies and updates are effective as of July 1st, 2023. You consent to using electronic communications and you agree that all notices, disclosures, agreements, etc. that we provide to you electronically (via e-mail and on the site) satisfy any legal requirement that such communications be in writing.

Cancellation/Refunding

​At this point in time, on-going services (meaning services with follow-ups included) cannot be cancelled or refunded after being purchased in whole prior to the beginning of services. In the future, this policy will change.

One time services can be cancelled/refunded, as long as it is done within 24 hours prior to the start of service. By accepting and paying for this service, you are agreeing to all of Simple City Wellness’s terms and conditions.

Location Based Care

At this point in time, our dietitians can only service people currently living in Wisconsin. With this being said, proof of Wisconsin residency will be required by the client before time of service.

Can all services be done virtually?

Yes. When you consent to using electronic communications and you agree that all notices, disclosures, agreements, etc. that we provide to you electronically (via e-mail and on the site) satisfy any legal requirement that such communications be in writing. As long as clients have access to a virtual meeting platform and a method of virtual communication, all services can be completed virtually. Certain services like weight and height monitoring allows for more room for error when completing via virtual report; therefore, some services rely on accurate and honest communication.

Can a service be cancelled and refunded after being purchased?

You understand that all information provide to simplecitywelless via virtual meeting applications or virtual communication will be private and protected under all circumstances.
